Original registrants can request an emergency copy of their registration file
be sent to them via e-mail. Requests are made from our secure customer login
area. Upon approval, registration files will only be e-mailed to the registrant's
e-mail address.
Requests for access to registered materials can only be made by the original
registrant* either from our secure customer login area or via notarized letter.
To request that a registration be withdrawn, original registrant must submit
a notarized letter via mail to:

* It is understood that registrant will submit materials to be registered using
his/her legal name. If registration is in the names of more than one individual,
notarized letters must be submitted by each registrant to withdraw registration.
If a registrant is deceased, proof of death and written consent of his/her
representatives or heirs must be submitted.
